The Hospitalization List

The SSA wants a list of all of the hospitalizations related to the disability. Yeah, like I could keep up with all of that.

I have the approximate dates of both my inpatient and outpatient hospitalizations from the past three years, but my CM had an even better solution. He had some paperwork that he simply addressed to the records departments of the various hospitals to which I’ve been admitted requesting the dates, and all I had to do was sign each sheet. Very nice.

Now I have to try to figure out all of the jobs I’ve held in the past 15 years. That, I think, will prove to be a lot tougher, especially since I had all kinds of different jobs at DePaul while I was still there for undergrad. But at least this process is finally happening!

    Robin, I’ve had a ton of temp jobs, too, but at least all I have to list are the temp agencies. I hope so, anyway, and probably only the ones that actually got me assignments.

    Thanks, April! Yes, we do need them. Like I left on a comment on Kentucky Girl’s blog recently, I am so due for a paraffin treatment for my hands! :d At least my mental health benefits will start up again in January, so that should help.

    KSF, it’s the government, so I guess it’s to be expected. My CM also told me that like most applicants, I probably won’t be approved the first time, but at least I know he’ll help me with the appeal process if necessary.

    KG, actually, my CM is a social worker. :) But yeah, this is going to be a real pain. When I was either 21 or 22 I had nine different jobs in one year! :o

    DB, oh, yes. Once all the paperwork is turned in, I’ll even have to go in for an interview so they can make sure that I really am a whack job and deserve the benefits.

    Oh, the job history, yes, only list what is absolutely necessary that they can find out. You want to have gaps where you couldn’t work due to disability. If you show you have all kinds of experience or could always work, they’ll try to say well “You can stuff envelopes” or “Scrub toilets”.

    Be sure when it asks why you left a job, say something that relates to your disablility, like, don’t say you left because you didn’t like it or it didn’t pay enough.

    My husband’s disability was physical, but he had to see the Psych, too, not sure why. He did really well on the tests, (I could not have done as well, especially memory tests!) so I don’t know what to tell you there.

    When the Psych pushed a pen and paper at my husband and told him to write two sentences that related to each other? My husband wrote, “I have a gun. Give me all your money.”

    Not sure how the Psych scored that…

    That’s a great story, annie. I’ll have to remember that one! :p I’m thinking that having had ECT should work in my favor. I hope.
